This is an online claim form. You can choose from the drop-down list the recipient of your claim. It is also possible to file a claim at the FMT’s registry office ( downloadable paper format here) or send it by email to the appropriate committee. Anonymous deposit is also possible. Your complaint will be processed as soon as possible, with transparency and objectivity.
